Express Scripts to fund $175,000 for Hospital’s CHAP partnership with Memory Care Home Solutions

Pilot program to benefit seniors with dementia and their family caregivers

Christian Hospital’s Community Health Access Program (CHAP) and Memory Care Home Solutions are pleased to announce that they have received $175,000 from Express Scripts to fund a pilot program that will enhance the health of seniors with dementia and their family caregivers. This partnership was brought to life by the leadership at Express Scripts who saw a unique opportunity for collaboration between two pioneering organizations providing quality community-based health care in north St. Louis County. This support underscores the incredible generosity and commitment of Express Scripts to access of quality health care. 

Both CHAP and Memory Care Home Solutions are working to reduce 911 calls, hospital readmissions and unnecessary emergency room visits for vulnerable populations. Through this collaboration, Christian Hospital case managers and advanced practice paramedics have received specialized training from Memory Care Home Solutions’ staff experts to identify patients for whom dementia is an underlying cause of their frequent use of 911 or emergency department care. CHAP will provide in-home medical care for these patients and assistance with community resources and Memory Care Home Solutions will deliver dementia care education and training to family members. By meeting the complex health needs of the patient and helping families cope with dementia symptoms, this project aims to keep older adults healthy at home and reduce stress for overwhelmed families.
