Stroke patients receive an interdisciplinary, individualized treatment approach to care from our dedicated team of health care professionals. While no two stroke patient treatment plans are exactly alike, most plans feature a set of rehabilitation goals that use a combination of therapies and activities designed to help patients improve physical mobility, regain daily living skills and prepare for community re-entry.
Depending on the patient's unique needs, the members of his or her interdisciplinary team may include:
- Case managers
- Certified rehabilitation nurses
- Clinical dietitians
- Occupational therapists
- Physical therapists
- Physicians with specialty training in physical medicine and rehabilitation
- Recreational therapists
- Respiratory therapists
- Speech-language pathologists
The stroke treatment team holds weekly meetings to discuss and evaluate each patient's progress and ongoing care. Primary care physicians and specialists continue to follow the patients on the unit.
